Does this remove metadata, or just let me change it?

It lets you set new values for title, author, subject, and keywords. To strip metadata entirely instead of editing it, use Privacy Risk Scanner's cleanup option.

Will this change how the PDF looks?

No — metadata is separate from page content. Only the document properties change, not anything visible on the pages.

Does my file get uploaded anywhere?
